ある日、田舎に住む少女、宮水三葉は、奇妙な夢を見る。
One day, Mitsuha Miyamizu, a young girl living in the countryside, has a strange dream.
目が覚めると、自分が都会の少年、立花瀧の体になっていることに気づく。
When she wakes up, she finds herself in the body of a city boy named Taki Tachibana.
一方、東京に住む瀧も、自分が三葉の体になっていることに驚く。
On the other hand, Taki, who lives in Tokyo, is surprised to find himself in the body of Mitsuha.
この奇妙な入れ替わりは、何度も繰り返される。
This strange switching takes place over and over again.
二人は最初は混乱するが、日記やメモを通じてお互いに連絡を取り合い、少しずつ状況に慣れていく。
The two are confused at first, but they keep in touch with each other through their diaries and notes, and gradually get used to the situation.
三葉は都会の生活に憧れ、瀧の学校や仕事を楽しむ。

一方、瀧は田舎の風景や三葉の家族、神社の伝統に触れ、新しい経験をする。

しかし、突然入れ替わりが止まり、瀧は三葉に会いたいという強い気持ちに駆られる。
But suddenly the switching stops, and Taki is driven by a strong desire to see Mitsuha.
瀧は三葉の住む町を探し始めるが、そこで驚くべき事実を知る。
Taki begins to search for the town where Mitsuha lives, and there he learns a startling fact.
その町は3年前に彗星の破片が落ち、消滅していた。
The town was obliterated three years ago when a piece of a comet fell on it.
三葉とその町の人々は全員亡くなっていたとされていたのだ。

混乱しながらも、瀧は神社の御神体を訪れ、三葉と再び繋がる方法を探る。
Confused, Taki visits the sacred body of the shrine and searches for a way to reconnect with Mitsuha.
やがて瀧は時間を超えて三葉と再び繋がる。
Eventually, Taki reconnects with Mitsuha through time.
彼は彼女に彗星の災害を警告し、町の人々を避難させるよう伝える。
He warns her of the comet disaster and tells her to evacuate the town.
三葉はそのメッセージを信じ、必死に行動するが、人々を救うことは容易ではない。
Miyo believes his message and acts desperately, but saving the people is not easy.
彗星の夜、二人はついに直接会う。
On the night of the comet, they finally meet in person.
しかし、お互いの名前を忘れないようにしようとする瞬間、入れ替わりが終わってしまう。
However, the moment they try to remember each other's names, the swap ends.
記憶が薄れていく中で、二人は強くお互いを覚えようとする。
As their memories fade, they try to remember each other strongly.
数年後、東京で暮らす瀧と三葉は、偶然すれ違う。
Years later, living in Tokyo, Taki and Mitsuha cross paths by chance.
お互いにどこかで会ったような気がしながら、名前も覚えていない。
They feel as if they have met each other somewhere, but cannot remember each other's names.
それでも二人は立ち止まり、振り返る。
Still, they stop and look back.
「君の名は？」
"What's your name?"
と問いかけながら、彼らの物語は新しい始まりを迎える。
